New Patients
Please arrive at least 20 minutes prior to your scheduled appointment with the following:● Insurance card.
● A valid form of identification.
● Pre-register on our patient portal below, or if you prefer, download our New Patient Packet and bring it with you to your appointment.
● Any relevant prior medical or cardiac records and bloodwork (Release of Information Form).
● Prescription medications or medication list.
● If you have any cardiac catheterization films or CT images to review during your visit, contact the department at the hospital where it was performed at to get the images on a CD (either the cath lab or radiology, respectively, not Medical Records).
Returning Patients
Please arrive at least 15 minutes prior to your scheduled appointment and notify the front office staff of any changes in your insurance, address, or contact information. Notify the staff of any need for refills or changes in pharmacy. Please bring a copy of the following:
● List of medications and doses.
● Blood pressure log, if applicable.
● A copy of any bloodwork performed elsewhere since your last visit.
● Any medical records from other physicians or hospitalizations.
